The Lebanon Community Health Nurses are regular part-time employees of the city who are part of a Mobile Integrated Health (MIH) Agreement with Dartmouth-Hitchcock Medical Center. The MIH Agreement was signed in December 2020 and is in effect until December 2023, when it will be assessed to determine if the program will continue for an additional three-year term.
The Lebanon Community Health Nurses provide support for the management of chronic disease which may be complicated by numerous issues which may require longer-term support. The nurses will provide guidance and education for those who need community resources to help them through an illness or crisis. The focus is on home visiting, teaching, coaching, integration of services, as well as prevention of disease and promotion of health for individuals and the community.
The Community Health Nurses use their nursing skills, experience, and judgment in evaluating an individual’s healthcare needs and may refer them to appropriate healthcare professionals and community resources and services. All duties performed are within the scope of the New Hampshire Nurse Practice Act and in collaboration with others.
